Plan Overview

The 4-Tier Plan was designed to assist members reduce the burdensome expense of prescription drug medications by enabling them to participate in their prescription drug choices. Members obtain access to the APS contracted drug pricing with over 56,000 participating pharmacies. They are provided with a comprehensive drug pricing formulary booklet that will enable them to work with their physician in selecting the most cost effective drugs under the plan. The formulary booklet organizes drugs by therapeutic class within a 4-tiered pricing matrix. This format will allow members, working with their physician, to easily identify alternative drugs when a more costly drug is prescribed. The primary benefits of the Optimum HealthCard 4-Tier Plan include:

• Tier One: Preferred Generics for $10 or less
• Tier Two:Preferred Brand Name and Generics for $20 or less
• Tier Three: Preferred Brand Name and Generics for $40 or less
• Tier Four: Non-Preferred Drugs at discounts of up to 50%
